Quality Assurance
At Arch Dental Laboratory we have high quality standards that involve the implementation of thorough quality assurance checkpoints throughout the manufacturing of your customhand-made products to reduce remake occurrences.
• Failure of a restoration due to an accident, neglect, changes in tissue or bone structure, and improper dental hygiene will not be covered.
• If we must remake due to insufficient information or records for example:
a new impression, new bite, Dr. to trim die due to unclear margins, no stump shade, wrong shade on RX resulting in remake, wrong information on the prescription and we were asked to proceed with the lab work, there will be a charge on the remake.
If a remake becomes necessary, the product will be evaluated carefully to determine what charge if any is fair. Please return all old impressions, models and crowns.
We stand behind our work and for any other reason than stated above all work is covered under our warranty.
Shipping Instructions
Arch will supply you with shipping materials, when you are running low call us at 205-979-1002. If you have any questions or need help scheduling your pick-ups, please call us. Please send scans to archdentalarts@gmail.com.
Pick up & Delivery
1. Fill out the prescription completely. Email photos to info@graydl.com
2. Place the prescription in the front pocket of the provided OSHA bio-bag.
3. Place models, bites and impressions in the OSHA bio-bag and seal the bag.
4. Place the OSHA bio-bag in a GRAY shipping bag or box.
5. Secure all implant parts in a closed container
6. If returning Diagnostic wax-ups or models they need to be wrapped in bubble wrap carefully to prevent damage during transport.
7. Call GRAY DENTAL LAB to schedule your pick-up at 205-979-1002

Credit Policy
TDA offers an open monthly account on all cases. Each case is invoiced and statements will be mailed around the first of the month. Statement balance is due within 20 days of receipt. A 1.5% service charge will be applied to any balance over 20 days past due. Customers with an outstanding balance over 90 days will be converted to a C.O.D. basis. Accounts that are sent to our attorney for collection will be responsible for all attorney fees and court.
